| Comment: | Stroke patients with large artery occlusion have a high risk of recurrent cerebral ischemia despite aggressive medical treatment. Surgical revascularization with extracranial-intracranial (EC-IC) bypass has remained a controversial treatment option. Over the last 2 decades, much has been learnt on the clinical and neuroimaging criteria for selection of patients with critical cerebral hypoperfusion most likely to benefit from EC-IC bypass, together with advances in operative techniques in this procedure.
